Using the Patient Portal (for Current/Established Patients)
Please note that the patient portal is for established patients only, and access will be granted when a new patient visit is scheduled.

Patients can communicate with Dr. Chu by sending a message through the secure patient portal. This is a part of the electronic health record and can be used for non-urgent clinical issues. I will respond to messages in 1-2 business days.

If you (or your child) are suicidal, fear that you/they will do harm to yourself/themselves or others, suspect that you/they are having a severe allergic reaction to a medication, or face a life-threatening emergency, please call “911” or go to the nearest emergency room. Please do not use e-mail or the portal to communicate urgent concerns.

The patient portal also serves as an important means to distribute and complete rating scales to aid in diagnostic assessment and/or follow progress throughout treatment. Assigned rating scales should be completed prior to scheduled appointments in order to best integrate this information at the time of the session.
